Membership Form
Please print and fill out the membership form and mail to the address you were given by the membership coordinator.

- Annual renewal and donations
Education discount: Adath Shalom offers its members a discount on membership fees for Jewish education costs of a member’s children.
The discount will be calculated as 75% of the total tuition paid for all children, up to a maximum of 75% of the total membership fee.
The discount will be valid for any Jewish school, program, activity or group that includes a significant educational component.
Yahrtzeit Information
We provide Yahrtzeit reminders to our members via direct email and in our bulletin, published 5-6 times per year.
Please use one of the following methods to send us your Yahrtzeit information.
Email option
For each person, please send the following information to memberinfo@adath-shalom.ca:
- Your full name and phone number
- Full name of deceased
- Relationship to you
- Date of death. If you don't know the hebrew date, please send the gregorian (english) date and we will determine the hebrew date for you.
We will confirm with an email or phone call.
